Test Request Form

You need to get Test Request Form on ETG Website.

It is processed by ETG.

EtherCAT conformance test contract

Before fill out it, read “General Conditions for the EtherCAT Conformance Test” and “Personal Information Protection Policy”. We think you assent conditions if you submit this contract.

1. Requesting Company

Test Date: Fill out the test day which you and ETC-J set.

Quotation reference number: Skip in blank. ETC-J fill out invoice number after you submit it.

Name of signee: Fill out the Date, Place, and Signature.

2. Device under Test (DuT) Details

Copy the table of “4 Device under Test (DuT) Details” in Test Request Form by items.

You can use screenshots of Test Request Form.

Device check list

Submit with DuT before the Test.

CTT project files (*.ctp) for the second time in a row about every listed device are also needed before the Test.
